If scheduling by phone, please ask the receptionist for your arrival time and appointment time, to avoid confusion.
Booking online is your actual appointment time with the provider, so please arrive 15 minutes early.
If a patient arrives more than 10 minutes late for an appointment, it is possible they may be asked to be rescheduled. We appreciate a 24 hour notice given if an appointment needs to be canceled.

For medical services, we currently accept most Blue Cross, Providence and Pacific Source plans. Any co-pays are expected to be paid on the day of service immediately before the appointment.
Patients with no insurance or with other insurance plans are always welcome and can schedule medical appointments with us on a self-pay basis. Ask receptionists for the self-pay fee schedule.

Often, a Botox® consultation and treatment can be done on the same day.
We request a deposit of $200 for brief cosmetic consultations scheduled with the nurses/Jamie NP.
The deposit can be used to pay towards the treatment, thus making the consultation complimentary if doing the procedure with us.
Minimum Botox® treatment is 20 units, which is $300. Botox® treatments using over 50 U are charged $12/unit which is $600.
We request a deposit of $240 for cosmetic consultations scheduled with Dr. Klein. This is a 30 minute appointment to review what is the best direction to take for your needs. This fee can be applied towards any procedure. If you do not show up for the consultation, the deposit is forfeited.
Voluma® and Sculptra® treatments require a $500 deposit for scheduling. Microneedling appointments require a $250 deposit when scheduling. These appointments take larger blocks of time to schedule, and multiple staff. Balance is due on the day of treatment.
Please remember to stop all blood thinning medications/supplements 1 week prior to procedures.
Anything that you want removed that is a benign lesion is considered “cosmetic surgery” by most insurance companies, meaning they will not pay anything for that procedure.
Examples of dermatology concerns that are often uncovered are lipoma removal, cryotherapy for seborrheic keratoses and the removal of some moles and cysts. We can provide these services for you, on a self-pay basis. Fees for these services are determined on an individual basis regarding their size and location, and can be determined at the time of evaluation.
Full payment is expected for cosmetic procedures on the same day of the service for most services such as Botox®, Juvederm®, lasers, chemical peels, and Hydrafacial®, and can be purchased in advance as a package.
Credit cards and cash are acceptable forms of payment. We do not accept checks.
