Please arrive 15 minutes prior to your scheduled appointment in case there is any paperwork that requires attention before the appointment.
If scheduling by phone, please ask the receptionist for your arrival time and appointment time, to avoid confusion.
Booking online is your actual appointment time with the provider, so please arrive 15 minutes early.
If a patient arrives more than 10 minutes late for an appointment, it is possible they may be asked to be rescheduled. We appreciate a 24 hour notice given if an appointment needs to be canceled.
When purchasing a package of services at one time to obtain a discount on the fees, those services must be used within 12 months from the time of purchase.

Cosmetic Appointments
For medical services, we currently accept most Blue Cross, Cigna, MODA, Aetna, Providence and Pacific Source plans. Any co-pays are expected to be paid on the day of service immediately before the appointment. We do not accept Medicare or Medicare Advantage plans.
Patients with no insurance or with other insurance plans are always welcome and can schedule medical appointments with us on a self-pay basis. Ask receptionists for the self-pay fee schedule. This is the fee for the office visit, additional procedures such as biopsies, excisions and cryotherapy may cost more.
We take deposits for appointments as follows:

Cosmetic Appointments
We request a deposit of $250 for cosmetic consultations scheduled with Dr. Klein. This is a 30 minute appointment to review what is the best direction to take for your needs. This fee can be applied towards any procedure. If you do not show up for the consultation, the deposit is forfeited.
Often, a Botox® consultation and treatment can be done on the same day. Botox® treatments are customized, and the fee is $300 and higher.
Voluma® and Sculptra® treatments require a $250 deposit for scheduling. Microneedling appointments require a $250 deposit when scheduling. These appointments take larger blocks of time to schedule, and multiple staff. The balance is due on the day of treatment. Ellacor requires a $250 deposit to schedule.
IMPORTANT: stop all blood thinning medications and supplements 1 week prior to procedures.

Anything that you want removed that is a benign lesion is considered “cosmetic surgery” by most insurance companies, meaning they will not pay for it. Examples of dermatology concerns that are often uncovered are lipoma removal, cryotherapy for seborrheic keratoses and the removal of some moles and cysts. We can provide these services for you, on a self-pay basis. Fees for these services are determined on an individual basis regarding their size and location, and can be determined at the time of evaluation.
Full payment is expected for cosmetic procedures on the same day of the service for most services such as Botox®, Juvederm®, lasers, chemical peels, Hydrafacial®, and Ellacor. Credit cards and cash are acceptable forms of payment. We do not accept checks.
