How Ellacor works
Local anesthetic done first
The patient feels minimal sensation during the treatment because a local anesthetic using lidocaine, similar to the dentist, is done first. Most patients rated it a 2 on a pain scale from 1 -10.
Hollow needles
Ellacor uses small hollow needles go into the skin all the way to fat creating the very tiny cores of skin, up to approximately 12,000 times, taking away 5-8% of the skin's area. It is a mathematical method of skin reduction. Then the ellacor vacuums the little micro-cores out, leaving full thickness holes.
Skin comes together
Within minutes to hours the holes close together and heal. Most of the healing happens in the first 3 days afterwards, and takes about a week, similar to microneedling. No sutures, just basic wound care afterwards.

How Dr. Marla Klein Thinks about Micro-Coring™
I'm so excited about Ellacor and here is why:
#1 : Thermal Energy is bad for the skin. (Ellacor is not thermal energy)
Thermal energy (heat from any source such as fire, the sun, radiofrequency and ultrasound) treatments are bad for the skin, especially when used repetitively.
#2 Fractionated lasers were an improvement over ablative lasers in terms of safety. The original CO2 ablative lasers worked well at removing the appearance of sun damage on the surface of the skin, but occasionally led to unacceptable scarring. The fractional lasers helped to stimulate collagen, but could not actually remove skin, like Ellacor can.
#3 Hyper-diluted lidocaine local anesthetic solution is good. I performed tumescent liposuction and facial fat grafting for years. I only stopped because I feel Sculptra is better. In that process, I learned about diluting lidocaine to safely anesthetize large surface areas, but decreasing the dose of lidocaine, making it safer for the patient.
#4 Sculptra is a collagen stimulating agent, but it doesn't work on everyone. About 85% patients are satisfied with Sculptra, and the others we call "non-responders". We needed something that can remove excess skin without leaving scars for those patients who do not want or could not afford a facelift surgery.
#5 Microneedling is great, but again because it requires the patient's body to respond to the treatment to make new collagen, it doesn't work on everyone.
#6 Tiny punch biopsies don't scar. As a dermatologist, I have performed thousands of punch biopsies. I noticed that the very small punch biopsies (1.5 mm) were not leaving scars behind on most people. The Ellacor machine uses hollow needles to remove even smaller (0.5 mm) cores of skin.
Since the start of 2023, we have been treating patients with Ellacor. The downtime after a treatment is about 1 week, and all patients have described as "easy" so far.
